New iRhythm data supports the use of long-term ambulatory cardiac monitoring (1)

iRhythm(Nasdaq:IRTC) today announced the release of four studies that strengthen the evidence behind its long-term cardiac monitoring.

San Francisco-based iRhythm presented the data at the Heart Rhythm Society's annual meeting, Heart Rhythm 2023, in New Orleans. The data supports ambulatory cardiac monitoring for up to 14 days with the company's Zio XT. It has demonstrated higher diagnostic yield and better diagnostic accuracy of ventricular arrhythmias in a variety of patient populations.

"These studies thematically demonstrate the value of iRhythm's Zio products and services – particularly in identifying ventricular arrhythmias for risk stratification and disease management,"thesedr. Mintu Turakhia, iRhythm's Chief Medical Officer, Chief Scientific Officer and Executive Vice President of Product Innovation. "The results have important clinical and scientific implications and further demonstrate the power of our technology across the full range of clinical arrhythmias."

Extended ambulatory ECG monitoring

iRhythm's first study, the EXAMINE-HCM study, evaluated patients with hypertrophic cardiomyopathy (HCM). He found that long-term monitoring of up to 14 days with Zio XT significantly improves the diagnostic yield of non-sustained ventricular tachycardia (NSVT).

Key outcomes included 48 hours of monitoring, in which nearly two-thirds of patients with high-risk NSVT episodes were absent. May affect risk stratification and treatment.

iRhythm showed a diagnostic yield of 18% for detecting NSVT in the first 48 hours. He observed 48% at 14 days, with 63% of first NSVT episodes occurring after the first 48 hours.

The company saw an 8% diagnostic yield in detecting high-risk NSVT episodes within the first 48 hours. This compared to 24% after 14 days, with 64% of the first high-risk NSVT runs occurring after the first 48 hours.

iRhythm premature ventricular contractions (PVC) strain analysis

The company performed a PVC burden analysis of 106,705 patients. It showed the association of increased monitoring for at least 7 to 10 days duration with greater accuracy in categorizing patients by PVC burden level.

iRhythm says the important findings show how misclassification can affect clinical decision-making.

An estimate of 48 hours of PVC misclassified PVC burden compared with 14 days of ambulatory ECG monitoring. At least seven days of continuous ambulatory ECG monitoring (performed by Zio) were required to achieve greater than 95% accuracy in load categorization.

Practical variation after NSVT ECG detection

A retrospective Veterans Health Administration study evaluated 58,279 patients. One-third of patients who received long-term ambulatory ECG monitoring for up to 14 days with Zio had NSVT.

iRhythm said the findings highlight an opportunity to create risk models and integrated treatment pathways after ECG monitoring.

The study observed wide variation at the site level in diagnostic testing and treatment across the VA health system following detection of NSVT. It included the proportion of patients with an echocardiogram, coronary angiography, exercise test, cardiology care and use of cardiovascular medications in the 90 days after outpatient follow-up.

Evaluation of flecainide in arrhythmogenic right ventricular cardiomyopathy (ARVC)

iRhythm conducted the Flec-ARVC study at the University of Rochester, the Johns Hopkins ARVC Program and other external institutions.

The company said it marks the first randomized clinical trial evaluating flecainide in ARVC. iRhythm said that using the Zio services highlights its PVC load measurement capabilities and its capabilities in clinical trials and research.

In the randomized, placebo-controlled crossover study of 41 patients receiving the Zio XT service for seven days, flecainide significantly reduced the CVP burden.
